FULL CONFERENCE REGISTRATION
Full Conference Registration includes: all three days of the Technical Conference, Exhibits, access to the Demo Suite areas, the 40 years of DAC proceedings, and a ticket to the Wednesday Night Party at Disney's California Adventure™ park.

EXHIBIT-ONLY REGISTRATION
Exhibit-Only Registration allows admittance to the Exhibit Halls and Demo Suite areas, Monday through Wednesday.

FREE MONDAY EXHIBIT-ONLY PASSES
Register on-line or call toll-free (800) 321-4573, before May 16, 2003, to receive a pass to visit the Exhibit Halls and Demo Suite areas, on Monday only, June 2, 2003.

STUDENTS
A special student rate applies to individuals who are members of IEEE or ACM and are currently enrolled in school. Students who register in advance must include a valid IEEE or ACM student membership number and a valid student ID. Students registering on-site must present a valid IEEE or ACM membership card and a Student ID. Student registration includes the 40 years of DAC proceedings and a ticket to the Wednesday Night Party at Disney's California Adventure™ park.

ONE/TWO-DAY REGISTRATION
One/Two-Day Only registration includes: the day(s) you select for the Technical Conference, all three days of the Exhibits, access to the Demo Suite areas, and the CD-ROM edition of the proceedings. One/Two-day Only registration does not include the Wednesday Night Party at Disney's California Adventure™ park.

Tickets for the party may be purchased for an additional $45.00 at guest registration.

TUTORIALS
Registration for full-day tutorials will be available in March and are offered on Monday, June 2 and Friday, June 6, 2003. You must register for at least one day of the Technical Conference to attend a tutorial. Tutorial registration fee includes: continental breakfast, lunch, breaks, and tutorial notes. Space is limited to the first 125 persons. If your tutorial selection is filled and you do not indicate a second choice on the registration form, we will refund the tutorial registration fee.

HANDS-ON TUTORIALS
Attendees must register for a minimum of an Exhibit-Only registration in order to be eligible to enroll in a Hands-on Tutorial. Due to the proprietary nature of the discussions, presenting companies have the right to refuse access to employees or contractors of competitors.

WEB REGISTRATION - CLOSED
A. Registration may be charged to Visa, MasterCard or American Express. Credit card payments require the complete credit card number, expiration date, name as it appears on the card, and complete billing address.

B. May 5, 2003, is the DEADLINE to qualify for the advance registration rate. Payment transactions must be completed on or before May 5, 2003, in order to receive the advance registration rate. The DAC office will continue to accept registrations until May 12, 2003, at the on-site rate. After May 12, 2003, all registration must be done on-site.

C. Refund Policy: Written requests for cancellations must be received in the DAC office by May 5, 2003, and are subject to a $25 processing fee. Cancellations received after May 5, 2003, will NOT be honored and all registration fees will be forfeited.

D. Membership number must be included at time of submission to receive the membership rate. No refunds will be made for change in membership status. If you would like information on becoming an IEEE or ACM member, visit ACM or IEEE .

A. Payment MUST be included with the registration form or it WILL be discarded.

B. Register ONE person per form (copy form as needed).

C. If payment is received from a non-US bank, the attendee will be charged a collection fee of $30.
